Ways of Urinary Catheter Insertion

There are two ways to place the urinary catheter:

  • Transurethral – Entry is through the urethra
  • Suprapubic – Entry is through the anterior abdominal wall

In both of these cases, both the simple (latex) and the silicone urinary catheter (100% silicone) can be used.

Cases requiring bladder catheterization

  • Urinary retention (acute or chronic)
  • Urine collection for tests
  • In patients whose fluid balance must be monitored, the control of the exact amount of fluid that the patient expels.
  • In the case of a recumbent patient, especially when there is an open wound, to avoid contamination from urine pooling.
  • After surgery
  • In case the patient needs bladder washings

Intermittent Catheterization

In some cases, where the patient cannot urinate on his own and does not have a permanent urinary catheter (foley), the procedure of intermittent catheterization is performed in order to decongest the bladder. We insert a sterile tube with aseptic technique to check the bladder remnant.

Contraindications for transurethral urinary catheter insertion

  • Acute prostatitis
  • Suspected urethral trauma
  • In case of urethral surgery or other urological procedures
  • In case of faecal incontinence, when the transurethral catheter gets dirty

In cases where there are any of the above contraindications, the use of a suprapubic urinary catheter is recommended.

Precaution of infections caused by the urinary catheter

  • Sterile materials and aseptic technique are required when inserting the urinary catheter
  • Frequent change of urinary catheter. Its use, in case a simple (latex) urinary catheter is used, should not exceed 20 to 25 days, while when a silicone one is used, 25 to 30 days.
  • Preserving a closed urinary system so that it is not colonized by bacteria.
